Client Portal

To directly message your provider, request an appointment, relay concerns, or for anything else, please access your portal or create a login using the link below.


1) If you are in crisis, or experiencing a life-threatening, psychiatric emergency, please call 911. You can also go to the nearest hospital emergency room, or call 988 to reach the National Suicide/Crisis Hotline,  Maricopa County Crisis Line at (602) 222-9444, EMPACT Crisis Line at (480) 784-1500, or the Suicide Prevention Hotline at 1(800)-273-8255.


2)  Refill requests should be submitted 7 days in advance to avoid lapses in treatment.


3) If you are an existing client reaching out to schedule, or make changes to an existing appointment, please do so by accessing your client portal. If you are having difficulties accessing, please call the main number at (602) 428-4780, and Anai can assist you. 


4) If you are needing documents completed for school, work, ect, please submit the request  directly through your client portal.  


5) If you are requesting medical records, please be advised that under HIPAA law, we are required to provide copies of your health information within 30 days of your request. Thank you for your patience in advanced. Records can be requested through your portal or via Fax. Our fax number is (602) 532-7021.


6) If you are a new client, or would like information on how to become a client, please reach out to Anai directly, her email . She can also answer any questions you may have in regards to costs, ect. and assist you with scheduling.  I look forward to working with you!
